Quiz plugin v2.0

Quiz plugin was fully re-designed and improved. Among others version 2.0  has the following changes:
- Improved HTML and CSS
- Compatibility with Elxis 5.x
- jQquery removed (now uses native javascript)
- You can set a colour and an icon (Font awesome) on each category.
- The time to answer a question is now category based meaning that now different categories can have different reply time limit.
