Customising IOSR 3 - extra fields on the booking form

How do I?:
1) remove VAT ID from booking form, or hide it?
2) add extra fields like second <textarea> and also yes/no radiobutton?
3) Have tickbox "I agree with Terms & Conditions" (and some text with it) and without ticking it booking will not go through?

I think I've must have got disconnected when we were speaking, I have tried again without any luck - maybe it is my ISP. Anyway..

These field I would like to put on the page where clients has "comment box" (textarea), I would like more fields there.
