there was a form builder for elxis: Facile Forms 1.4.7, but it works only with oldest versions (elxis 2006.x).
https://www.elxis.org/legacy-extensions.html (https://www.elxis.org/legacy-extensions.html)
take care to use it, because the component can have problems in the new elxis versions.
the best solution: create by yourself the form you need it. is simply. in internet you can find many php examples to create the forms  ;).

Have a look at http://www.jotform.com/ (http://www.jotform.com/) where you can create online a form and the only one you have to do into your elxis site is to insert a given url. The form is appearing into your elxis site like wrapped url but is not executed into your site.

There will be a component for Elxis CMS which you will manage the form as you like.

I have a bot named eForms under development that can generate and execute any form. I believe it will be ready in 2 weeks.
Development progress can be seen here: https://www.isopensource.com (https://www.isopensource.com) (in the right column)

Title: eForms 1.0 released

eForms 1.0 bot was released and you can download it from EDC (http://www.elxis-downloads.com/downloads/communication/280.html). With eForms you can create and process a variety of HTML forms within Elxis content items and autonomous pages. The form's elements are stored inside an XML file.

You will find eForms documentation (http://wiki.elxis.org/wiki/Eforms) on Elxis wiki.

Enjoy!
